Visitor Request Form

Short Term Visitor Request (1- 59 days)

Instructions:
• Using a computer, internet enabled device, fill out sponsor and visitor information.
• You will receive a QR code which can be printed or presented on your device at any Welcome Center.
• Sponsor and Visitor must both be present at time of issuance.
• Visitors are required to fill out a criminal history request form.
• Note: If unable to pre-enroll we will gather information at time of visit.

 

Long Term Visitor Request Form (Longer than 60 days)

Visitor Request Form

Instructions:
• Fill out the form completely with both the sponsors and visitors' information.
• A commanding officer or deputy must digitally sign the member’s request for visitor passes prior to visitors’ arrival.
• In the event the commanding officer or designated representative is unable to digitally sign visitor request form, a wet signature is authorized as long as DOD ID # is provided.
• Forms can be emailed to the visitor center organization email via the link above.

Unofficial Foreign Visitor Request

All unofficial foreign visitors wishing to access JB MDL must undergo additional background checks and processing before being issued a visitor's pass. This includes non-U.S. citizens who hold a foreign passport and are not a U.S. permanent resident or do not have Department of Defense identification that would otherwise grant them base access. The processing and additional background checks can take up to 10 additional days to complete, receive approval and the visitor to be issued a pass. Contractor Request Forms

Forms must be submitted 72 hours prior to visit

SFFL 4/Contractor Request Form

  • Fill out form completely
  • Return form with a digital signature.
  • If you send us the form in an email, subject the email with the title of the form and date of request. Example: (Cantu Services/01Jan) etc.
  • You can fax, hand carry, and email request forms with digital signatures (609) 754-3854 (Fax)
  • Forms with wet signature must include your DOD ID #, those must be hand carried and dropped off at the visitor center by the person signing the request form.  
  • Wet signatures will not be allowed via scan, email or fax.

Note: Inform contractors/visitors who will be sending the request form. This allows the center to better process your visitors.

Retirees/Non-Base Affiliated Housing Member

• Retirees and Other Eligible Tenants residing in privatized housing who are authorized to sponsor may request short term passes not to exceed 60 consecutive days. Anything longer than that must be coordinated with United Communities or BBC and approved by the 87 CEG/CC or designated housing representative.
• Other Eligible Tenants residing in privatized housing with locally issued credentials (DBIDs)
Are authorized to sponsor individuals. They can request short term passes for visitors not to exceed 60 consecutive days. They must be limited to their particular housing area only. Anything longer than that must be coordinated with United Communities or BBC and approved by the 87 CEG/CC or designated housing representative

Social Gatherings/Special Events

Special Event Form

Instructions:
• Special event requests must be submitted 72 hours prior to event.
• DOD Card Holder approval is required.
• If your event is greater than 100 members, the Special Event must be authorized by the 87 ABW Commander.
• All fields are required, this includes names, full social security numbers, and DOB
• Last page of special event form requires all fields to be initialed for sponsor to assume responsibility of guests and acknowledgement of procedures.
• Special events are only for groups of 5 or more. (less than 5 are case by case)
Please call once you have completed and emailed it back to ensure that we received it.

Veterans

As of 1 January 2020, Veterans with a verified disability below 100 percent as well as a Veterans Health Identification Card (VHIC) marked "Service Connected," "Purple Heart," or "Former POW," will have access to the commissaries, exchanges, and some MWR activities. VA registered primary caregivers of eligible veterans will also have access. Upon first visit to the installation, eligible veterans and caregivers must present their VHIC and official caregiver letter at the Visitor Control Center (VCC), along with a government issued ID or U.S. Passport.

  • If you are a veteran and would like to bring a guest on the installation, the only authorized guests at this time are spouses and care givers. You must either bring your marriage license or VA care giver doctors note.
