Why do I need a yellow card?

If you have visited the 305th Medical Group, you may have noticed signs at the check in areas, laboratory, radiology or even the pharmacy asking for your Yellow Card. This is due to a congressionally mandated program, the Third Party Collection Program. It is not optional; it's the law -- Public Law 101-510 (10 U.S. Code 1095) states that all non-active-duty patients must have a current insurance registration form (DD Form 2569) and it must be updated annually. Upon completion of the form, a Yellow Card is issued to help quickly identify if you need to complete a new form.

How does this benefit me?
The direct benefit is you're allowing your private insurance to work for you. You are using the benefits you are entitled. If you have an annual deductible the money we collect goes toward your deductible. You will not be billed for this program, nor will your insurance premiums go up. You will receive an explanation of benefits from your private insurance, which lists services received during your visit at the 305th MDG. The explanation of benefits further discloses what your insurance company has paid and has not paid. You are not responsible for any charges.
Indirectly, the funds received from your insurance company are used to fund medical equipment, supplies, furniture, renovations and medical education for the clinic.
Last year the clinic recovered $354,000 from insurance companies. The clinic's goal this year is $1,100,000. You can help us meet this goal by updating your insurance information annually.

How do I obtain a yellow card?
On your next visit to the 305th MDG, stop by the Other Health Insurance kiosk by the pharmacy pickup window and complete the DD Form 2569. You can also ask for this form at your next medical appointment at the 305th MDG. Please make sure you have your insurance card.
